Registration

After scheduling an appointment, you will receive a registration link from eDerm. Login with the patient’s last name and date of birth. If you didn’t receive the email, please check your Spam Folder. If you still can’t find the email or can’t login, please call the office.
Online Scheduling: Currently, only established patients can schedule online.

New Patients:
All New Patients must complete their New Patient Forms PRIOR to scheduling an appointment.
New Patient forms are found on our website home page under patient forms. We ask that you print and complete these forms and drop them off at our office. We can also mail the forms to you or email them to you for your convenience.

New Patient froms can be returned via mail, dropping them off at our office or send them via email to IDGMontrose@mydermgroup.com.
If you are a new patient, we ask that you arrive 20 minutes ahead of your appointment.

Please confirm that with your insurance prior to appointment that your plan is in network: Rocky Mountain HealthPlans / Monument Health Group ID: COONEX.
We do not participate with: PHCS, CIGNA Connect.
